Rhinocraft 2026 — The Hybrid and Rib Edition

About This Course
Rhinocraft 2026 is Dr. Enrico Robotti’s live surgery course on achieving natural, unoperated-looking results in primary and revision rhinoplasty. Six unedited cases, more than 21 hours of operative video, show his hybrid method: structural and preservation principles used together rather than treated as rival schools.
Three primary cases cover dorsal preservation, tip management and functional restoration, with piezoelectric and powered instrumentation used for osteoplasty and osteotomies. Three revision cases cover rib cartilage grafting the way Dr. Robotti approaches it, restoring aesthetics and function without excessive bulk, including harvest, shaping, laminating and the SPF-SPLF constructs for an aesthetic dorsum. Each case is narrated, so the reasoning behind every intraoperative decision is explicit.
Techniques Covered
Cartilaginous push down, full let down, and modified push-down with partial bony cap preservation
Structural tip management with preservation and reconstruction of ligamentous support
Subperichondrial and subperiosteal dissection of the tip and dorsum
The three-point compartmentalization technique for reducing edema and refining surface contour
Sublaminar septal dissection and septal fusion sling suspension
Combined piezoelectric and powered instrumentation for osteotomies and osteoplasty
Septoplasty and turbinate management, including piezo turbinoplasty
Rib harvest, preparation, shaping and laminating for revision rhinoplasty
Safe dissection of the scarred revision tip and dorsum
Rib septal extension grafts, lateral crural strut grafts and sidewall reshaping
Transosseous sutures for added stability, and current SPF-SPLF graft variations
Rib plaster techniques for contour refinement and camouflage
The Six Cases
CASE 1 — PRIMARY, MULTIPLE DORSAL ASYMMETRIES
47-year-old female with contour irregularities, marked tip bulbosity, hump, excess tip projection and functional issues. Direct columella shortening, modified dorsal split, osteoplasty, high strip dorsal reduction, cartilaginous push down, septoplasty, septal fusion sling, medial crural transection overlap, full lateral crural transposition, dome-refining sutures, columellar strut and mini alar contour grafts.
CASE 2 — PRIMARY, HUMP AND WIDE TIP
27-year-old female with dorsal hump, excess volume and a wide tip. Modified dorsal split, osteoplasty, sublaminar septal dissection, high strip dorsal reduction including part of the bony cap, three-point compartmentalization, septal extension graft, fusion sling, slide under of the lower lateral cartilages, lateral crural steal, dome-defining sutures and alar wedges.
CASE 3 — PRIMARY, THIN SKIN AND DROOPY TIP
34-year-old female with thin skin, dorsal hump, droopy tip and a deviated septum. Modified dorsal split, osteoplasty, sublaminar septal dissection, high strip full let down, three-point compartmentalization, double septal extension graft, fusion sling, turn under of the lower lateral cartilages, lateral crural steal, dome-defining sutures, diced cartilage and alar wedges.
CASE 4 — SECONDARY, SADDLE AND OPEN ROOF
22-year-old male with thin skin, severe tip deformity, open roof, dorsal saddling, pollybeak and severe functional compromise. Burr osteoplasty with piezoelectric selective osteotomies, full rib L-strut reconstruction, multiple tip grafts, perichondrium grafts and an SPLF graft to the dorsum.
CASE 5 — TERTIARY, OVERROTATION AND ALAR RETRACTION
33-year-old female with tip overprojection, severe overrotation, alar retraction and dorsal saddling. Septum lengthening with a composite graft of perforated PPE and septal cartilage, full L-strut reconstruction using rib extended spreaders and a vertical rib element fixated to the spine, full tip reconstruction with extended and transposed lateral crural struts, interpositional grafts, perichondrium grafts to the tip and an SPLF graft to the dorsum.
CASE 6 — POST-SEPTOPLASTY, TIP DROP AND DEVIATION
41-year-old male after septoplasty, with tip drop, axis deviation and severe functional issues. Osteoplasty, Cottle septoplasty, full L-strut reconstruction with laminated rib grafts, modified dorsal split, spare roof type B dorsal reduction, septal extension graft and multiple tip sutures.
