About This Series

Step into the operating room with Dr. Ziad Katrib for a comprehensive operative video series covering primary rhinoplasty, revision rhinoplasty and complex nasal reconstruction. Across three volumes, 10 cases and more than 23 hours of narrated footage, the series sets out the structural principles, operative strategy and decision-making behind Dr. Katrib’s systematic approach.

The emphasis is on careful analysis, structural integrity and preservation of the soft-tissue envelope. The cases move from structural primary rhinoplasty, through complete reconstruction of the revision nose with rib harvest and structural grafting, to multistage forehead flap reconstruction of large nasal defects involving support, lining and contour.

Volume 1 — Primary Rhinoplasty: A Structural Approach

4 cases · over 8 hours of operative video

Nasal analysis, septoplasty, donor rib spreader graft reconstruction, advanced tip reshaping and long-term support strategies for natural aesthetic and functional results.

Case 1 — Crooked wide dorsum, large dorsal hump, deviated septum, bulbous tip
Component dorsal hump reduction with osteotomies and nasal bone suture fixation, septoplasty with removal of a large septal spur, midvault reconstruction with donor rib spreader grafts, folded caudal septal extension graft with ethmoid splint, depressor septi nasi and PRF membrane camouflage, suture-only tip refinement.
Case 2 — Facial and nasal asymmetry, dorsal hump, thin skin, plunging tip (tension nose)
Component hump reduction with osteotomies, full septoplasty, donor rib spreader grafts, folded caudal septal extension graft with ethmoid splinting, tip reshaping with lateral crural steal, medial crural overlap and cranial tip suturing, PRF membranes for camouflage.
Case 3 — Wide nasal bones, dorsal hump, low radix, wide bulbous tip
Hump reduction with precise osteotomies to narrow the bony pyramid, septoplasty, donor rib spreader grafts, folded caudal septal extension graft, tip sutures with lateral crural steal and medial crural overlap, depressor septi nasi radix graft, and intraoperative Kenalog application to limit swelling.
Case 4 — Subtotal septal perforation with severe saddle deformity
Harvest of the seventh, eighth and ninth ribs with perichondrium intact, reconstruction of the absent caudal septum with a ninth rib graft, release of the vestibular skin around the pyriform, eighth rib dorsal graft fixated to the nasal bones, tip suturing and diced rectus fascia.

Volume 2 — Revision Rhinoplasty: A Systematic Approach

3 cases · over 9 hours of operative video

Rib harvest, structural grafting, dorsal augmentation and complete tip reconstruction, with the focus on restoring support, airway function and natural form.

Case 1 — Saddle nose, columellar and nostril asymmetry, pollybeak, wide flat tip
Seventh and ninth rib harvest, rectus fascia and free perichondrium, full septoplasty, ninth rib septal extension graft, raised spreaders, total tip rebuild with lateral crural hinges and replacement grafts, camouflage, and supratip scoring and control sutures.
Case 2 — Short nose, crooked nasal bones, over-rotated tip, retracted columella
Rib, fascia and perichondrium harvest, asymmetric osteotomies with unilateral extended spreader grafting, ninth rib septal extension graft, lateral crural strut grafts for tip and external valve reconstruction, and upper dorsal augmentation with perichondrium and ultra-diced fascia and cartilage.
Case 3 — Wide nasal bones, extreme saddle nose, severe tip over-rotation
Eighth and ninth rib harvest, large bony movements with power tools and hand saws, reconstruction of the absent caudal and dorsal septum with raised notched spreaders and a ninth rib septal replacement graft anchored to the nasal spine, total tip reconstruction and camouflage of tip, bridge and midvault.

Volume 3 — Forehead Flap Nasal Reconstruction: Principles and Practice

3 cases · over 6 hours of operative video

Two-, three- and four-stage forehead flap reconstruction based on the principles of Dr. Fred Menick: defect analysis, structural grafting, airway preservation and staged reconstruction strategy.

Case 1 — Three-stage full-thickness paramedian forehead flap
Subtotal defect of the tip, dorsum and soft triangles after oncologic resection, shown in three videos: defect re-creation, rib columellar strut, tip, alar rim and tip grafts, intermediate re-elevation and thinning, pedicle and medial brow management, inset and tacking sutures.
Case 2 — Four-stage folded paramedian forehead flap
Subtotal defect of the tip and lower dorsum with a full-thickness alar defect, shown in four videos: rib structure and alar batten graft, folded flap for internal lining, wound matrix for the donor site, delayed ear cartilage batten, staged thinning, pedicle management, nostril retainers and alar crease re-creation.
Case 3 — Two-stage forehead flap for tip subunit reconstruction
Subunit principle for a defect of more than half the nasal tip, shown in two videos: subunit excision, tip suturing, thin two-stage flap, pedicle and brow management and tacking sutures.
